In consideration of being permitted to participate in the activities of Hands On Central Texas (HOCT), I hereby agree, for myself, my child or ward, my heirs, assigns, executors and administrators to release and discharge Hands On Central Texas and its officers and directors, partners, employees, agents and volunteers from all claims, demands and actions for injuries or death sustained to my person, my child or ward and/or damage to or destruction, loss or theft of my property or that of my child or ward and to the property of others as a result of my involvement in such activities, whether or not resulting from my negligence or the negligence of any other individual, or from accidents without negligence, or from the intentional action of other individuals, and I agree to release and hold Hands On Central Texas and its officers and directors, partners, employees, agents and volunteers from any cause or action, claim or suit arising therefrom.  I hereby attest that my attendance and involvement in such activities is voluntary, that I am participating at my own risk, and that I have read the foregoing terms and conditions of this release.

I hereby confirm, represent and warrant that I have never been convicted of or charged with a violent crime, child abuse or neglect, child pornography, child abduction, kidnapping, rape or any sexual offense, nor have I ever been ordered by a court to receive psychiatric or psychological treatment in connection therewith.

I grant full permission for Hands On Central Texas, its nonprofit partner agencies, and its officers and directors, partners, employees, agents and volunteers, to use photographs, video and audio of me, my child or ward, and quotations from me, my child or ward, in accounts, promotions and publications of this event and these activities, and I hereby waive my right of publicity in connection with such uses.  I further recognize that I will not receive any compensation in connection with the use of my likeness.

I attest that my attendance and involvement in such activities, and that of my child or ward, is fully voluntary, that I and/or my child or ward, are participating at my/their own risk, that I am over the age of eighteen, competent, and that I have read the foregoing terms and conditions of this document.  By signing below, I am agreeing to all stipulations as stated above.
